Overhead cable triceps extension: proper form, muscles worked and common mistakes

How to do it
Facing away from a high pulley with the rope overhead and the elbows pointing forward, extend the arms fully without letting the elbows flare. Works the long head in a lengthened position for greater stimulus.
Muscles worked
Main muscle: triceps. When it plans weekly volume, every set of this exercise counts in full for triceps: the generator does not split it with other muscles.
Overhead cable triceps extension or pushdown: which one loads the stretch
In the overhead cable triceps extension your arm sits by your ear, which leaves the long head of the triceps stretched as the rope goes back. In the triceps pushdown, with your arm at your side, that muscle works in a more shortened position. In practice, you feel the stretch at the back of your arm here and the triceps works with less weight, while on the pushdown you can load more and the peak tension arrives at lockout. Compared with the EZ bar skull crusher or the overhead dumbbell extension, the cable's advantage is that the resistance never drops away at any point.
Choose it as your second or third triceps exercise, after a press or dips, when you want extra work in a lengthened position. It helps if skull crushers bother your elbows, because the rope lets your wrists rotate freely. If it is hard to raise your arms overhead, take a split stance with one foot forward or lean your torso forward a little. The loads will be well below what you use on the pushdown, in ranges of 10 to 15 reps.
A detail specific to this exercise: when you reach the top, spread your hands and open the rope slightly to finish squeezing the triceps, but without letting your elbows flare. Keep your ribs down so your lower back does not arch.

Recommended sets and reps
As an isolation exercise, it works best in 3–4 sets of 10–15 reps with 0–2 RIR. Use it at the end of the session to add volume without systemic fatigue.
Common mistakes to avoid
- Flaring the elbows out, shifting tension away from the triceps to the chest and shoulders.
- Letting the weight stack touch down between reps: the cable loses its constant tension, which is its whole advantage.
- Cutting short the part of the range where the muscle is stretched: that is exactly where this exercise gives the most stimulus.
What the research says
This is an isolation exercise. For beginners, adding it on top of compound lifts does not always help: in the trial by Gentil and colleagues (2013), people who only did lat pulldowns gained as much arm size as those who also did curls. It earns its place with experience, and for muscles the big lifts leave short.
Its peak tension comes with the muscle stretched, and that matters: in the trial by Maeo and colleagues (2021), seated leg curls, which stretch the hamstrings more, built more muscle than lying curls, and Pedrosa and colleagues (2022) saw something similar in the quads when training the long end of the range.
On how hard to push: the meta-analysis by Refalo and colleagues (2023) found no clear advantage in going to failure over stopping just short of it. That is why the generator programs it with reps in reserve (RIR) instead of asking for failure on every set.

Overhead triceps extension: the one that grows the arm most
The long head of the triceps originates on the shoulder blade, so it stretches when the arm goes overhead. Maeo and colleagues measured the effect in 2023 in the European Journal of Sport Science: twenty-one adults did cable extensions for twelve weeks, one arm overhead and the other by the side, with the same relative load.
| Overhead | Arm by the side | |
|---|---|---|
| Long head | +28.5% | +19.6% |
| Lateral and medial heads | +14.6% | +10.5% |
| Whole triceps | +19.9% | +13.9% |
The arm trained overhead grew about 1.4 times more across the whole triceps (Maeo et al., 2023). And not only in the long head, as expected: in the other two heads as well. It did so while moving less weight, 34 to 39% less.
That is why there is no need to chase load on this extension. Go for the full range, with the elbow well bent at the bottom, and save the heavy loads for close-grip presses and dips.

Frequently asked questions
What muscles does Overhead cable triceps extension work?
Overhead cable triceps extension mainly works the triceps muscle group, plus synergist and stabilizer muscles depending on the variation.
How many sets of Overhead cable triceps extension should I do?
Between 3 and 5 sets per session, adjusting the total weekly volume for triceps to your level (about 10–20 sets per muscle per week).
Why does the overhead cable extension feel harder with less weight?
Because with your arm overhead the long head of the triceps starts from a stretched position and works under tension from the first inch. The cable also gives you no rest at any point. That is why it responds to lighter loads than the pushdown. You do not need to go heavy: prioritize full range and control.
